Laparoscopic splenectomy

Abstract
Background : Laparoscopic cholecystectomy has been accep ted as a standard procedure from all. the world. As better laparoscopic devices are introduced, more difficult cases will be challenged. Traditionally, splenectomy has been performed through a left subcostal -incision about 20-30cm, but in this time laparoscopic splenectomy has been accepted despite to some difficulties of anatomic problems.

Method : We experienced 6 cases of laparoscopic splenectomy which were safely and successfully done, From 1997. May to 1999. July, in Dong-A hospital, 6 cases of laparoscopic splenectomy was done. 4 of 6 cases were ITP (Idiopathic thrombocytopenic purpura), others were Hemolytic anemia and Hereditary spherocytosis, and these two. cases were accomplished with laparoscopic cholecystectomy.

Result:There was no case of conversion procedure. The average amount of blood loss is 450ml, the average operation time is 3hr 20 min. The duration of postoperation admission is 4 days. In ITP, the preoperation average count of platelet is 56000/mm3, and postoperation one is 244000/mm3.

Conclusion:The benefits of laparoscopic splenectomy were as followed. Fast recovery, less pain, cosmetic effect, early returning to normal social activity. For this reasons, we thought that laparoscopic procedure would be gold standard method of splenectomy in selected cases sooner.
